concurrent connections?

Does anybody have a tool that 'tracks' concurrent connections?

 

Specifically, I was interested to track the number of outstanding
connections to a given server or service during a particular segment in
time.  It's mostly a matter of looking at the timers and counting, but I
was wondering if anyone had already done something like it..  I will of
course be interested in 'failing' connections during the same period..
